Skill Assessment

Our home health team is trained to properly perform assessments on our clients’ health status and their current skills or capabilities to determine their needs and the services they can access to improve their conditions. We can also inform the client’s physician and family members about the changes in their needs so that they can stay comfortable and continue their recovery.

We also provide:

• Ongoing comprehensive assessment of the patient’s physical, psychological, and environmental needs
• Admission and subsequent visits as necessary. The scope and intensity of assessment shall be determined by the patient’s diagnosis, condition, needs, and desire for care and services settings
• Initiation and revision of patient’s plan of care in collaboration with the patient’s physicians as necessary
• Direct provision of services requiring substantial and specialized nursing skills.
• Initiation of appropriate preventive and rehabilitative nursing procedures
• Preparation of clinical records and progress notes
• Consultation and education of patient and family regarding disease process, self-care techniques, and prevention strategies
• Coordination of services between patient and case monitoring agencies on eligibility for services
• Referral to other professionals as needed
• Communication with patient’s physician and other home health employees regarding changes in patient’s condition
• Informing the physician and other employees of changes in patient’s needs
• Evaluating the effectiveness and outcomes of care
• Quality assurance interactions and supervision of aides rendering service
